Before You Arrive (Pre-Visit Checklist)

• Parking: Convenient parking is available right on the side of our building.
• Arrival Time: Please arrive 10 to 15 minutes prior to your scheduled appointment to allow ample time for check-in and paperwork.
• Prior Surgeries: If you have had any prior surgeries, please notify our staff before your appointment day.
• Implants & Medical Devices: If you have an implant or any medical device, please inform our staff before your appointment day and remember to bring the device identification card with you.
• What to Wear: * Dress in comfortable, loose-fitting clothing.
• No Metal / No Jewelry: Many imaging scans are highly sensitive to metal. Please avoid wearing jewelry, watches, or clothing with metal zippers, snaps, or buttons.

What to Bring With You

Please ensure you have the following items ready to present at the front desk upon arrival:

• Valid Photo ID (Driver’s license, state ID, or passport)
• Your Prescription (Rx) or referral form from your doctor
• Insurance Card(s)
• Implant / Medical Device Card (if applicable)
